PS Medical Outreach Case Mgr

Responsible for the coordination of supportive services and prevention-focused patient care in a fast-paced multidisciplinary clinical setting. Provides HIV prevention navigation and case management services aimed at increasing access to and retention in HIV prevention services, including Pre-Exposure Prophylaxis (PrEP) and Non-Occupational Post-Exposure Prophylaxis
(nPEP).
Serves as a primary point of contact for individuals seeking HIV prevention services and supports patients in accessing prevention-related medical care, medications, insurance coverage, pharmaceutical assistance programs, and community resources. Assists clients in overcoming barriers to prevention services and works collaboratively with patients to develop the health system literacy, self-advocacy skills, and resource knowledge necessary to successfully engage in ongoing prevention care.
Provides education, advocacy, support, and empowerment to clients as they navigate healthcare and social service systems. Promotes long-term engagement in prevention services through ongoing follow-up, care coordination, and linkage to supportive resources.
Serves as a liaison between the University of Utah HIV Prevention Program and community referral partners. Develops and maintains collaborative relationships with healthcare providers, public health agencies, community-based organizations, educational institutions, and other referral sources to strengthen access to HIV prevention services throughout Utah.
Supports outreach, volunteer engagement, prevention education, and community partnership activities designed to increase awareness of HIV prevention services and reduce barriers to care.
